Payroll – Benefit Forms

*All ORIGINAL SIGNED FORMS must be returned to Marisa Forchione, Payroll/Benefits Dept., with the Open Enrollment Form. 

Open enrollment forms only need to be completed if you would like to make a change to your current information. 

Even if you only want to change the number of pays you receive each year, 22 vs. 26, please complete the appropriate  enrollment form (AFSCME, WTA, Central Office, WAA.) below:

Please complete all of the above forms electronically.  Once completed, please print them, sign them, and then send them to Marisa Forchione in the Payroll Dept. via interoffice mail or via postal mail to Weston Public Schools, Payroll Department, 24 School Rd, Weston, CT 06883.
